You're correct, I've tested it once more with b_fixedphysics 1 and b_fixedphysicsfps 125 (which is set on Hirntot, afaik) and didn't experience the same issue again.
Every frame ET checks if there is any mouseinput, if yes spread goes up if no spread goes down. Isn't this complicated by the fact ET doesn't use raw mouse input? I can't see any preshooting nor aiming through walls except few moments, which could be caused by lag distortion in demo. Seems like a skilled player and if he uses something, hides it very well. On the other hand, considering his skill, I'm suprised he fails to time a nade correctly so often. :>
